Please send in a letter of application or complete the application form to the Chair of the PRG - Jane Horn, this can be posted, emailed or left at the practice marked for the attention of the Patient Reference Group.
The Application Form can be completed online using Acrobat Reader (Apps are available on Google Play, Apple Store and Microsoft Store), stored in a convenient location and emailed to us or printed.
